Medicare with a commercial secondary, Blue Cross Blue Shield, Optum, United Health Care/Commercial Plans

Denise Maynard

Denise does NOT accept insurance at this time. A superbill can be provided for out-of-network services. The superbill can be submitted to the insurance company for potential reimbursement.

Should one of our licensed providers not be in your insurance company’s network, they are glad to provide a superbill for your potential reimbursement.

This includes the initial interview, eight individual sessions, and workbook.

$630 due at the time of scheduling initial appointment and $630 due at the start of the 4th appointment.
